If you have information concerning a tax underpayment that you would like to report, we ask that you fill out the case evaluation form below so that we can properly analyze your potential case and any issues that may arise in your case. You do not have to respond to every question, but the more detail that you provide the better we will be able to advise you and assess our interest in representing you in the case.  The questions set forth below are designed to elicit information pertinent to assessing a tax underpayment case. If you would rather write a narrative, please address the issues raised by the evaluation form.

All information submitted to us during the evaluation process is protected by the attorney-client privilege, meaning that, by law, we cannot and will not disclose it to any third party, including the IRS, without your express permission. However, in filling out the evaluation form, we do not need the names of taxpayer individuals or entities in order to initially evaluate the claim.
